I am Jacinda Stroud (she/her). As an experienced Nurse Practitioner, who is also a personal health consultant and patient advocate, my mission is to provide clients with: • A better understanding of their health and the care connected to it. • Improved context for healthcare decisions. • A safe space to talk through their health priorities and guidance on how to best achieve them. I do this through information, collaboration, and inclusion. I am an experienced Registered Nurse (RN) and board certified Family Nurse Practitioner (FNP-BC). I have worked in healthcare since 2009, and have experience with newborn babies to ages 90+. I have worked in hospitals, outpatient clinics, residential care, emergency departments, operating rooms, and beyond. I customize services to the individual needs and priorities of my clients and their budgets/resources. I can also include the people in your support system, such as family or chosen family. I can meet with you virtually, at your home, accompany you for medical appointments or hospital admissions/discharges. I offer various levels of support appropriate to these settings. Based on your wishes, I happily collaborate with outside providers and other medical experts. I am trauma informed, LGBTQI+ inclusive, and serve all ages and stages including pregnant folx. A Bit About My Approach:

My goal is to empower my clients to successfully navigate the healthcare system with their best interests at heart. I listen to individual's goals, concerns, fears and questions and collaborate with them to develop a plan that supports them in the short- and long-term. I CAN ASSIST WITH: • Connecting/ communicating care between loved ones, who can’t physically be in the same place • Decrease anticipatory fear/ discuss what to expect BEFORE and AFTER planned hospitalizations, birth of a child, surgery or other scheduled events • Navigating medical appointments • Helping you prioritize, track and organize your care • Deciphering medical jargon • Setting-up a home routine after recent hospitalization • Helping you organize questions for an upcoming medical appointment I also support and ensure your voice, opinions, questions and concerns are heard and addressed by your doctors and other medical providers. Additionally, I can significantly improve communication between my clients and their outside healthcare providers/team. This: • Reduces risk of errors in medications, missed follow-up appointments, and other points of frustration. • Improves access to appropriate care and adjunct services, as well as more efficient use of time in scheduled appointments.

What To Expect From Our Intro Call:

Our intro call gives me an opportunity to learn about you and your primary questions/concerns. In most cases, by the end of the consultation we will be able to determine whether we are a good fit for one another, and to propose a formal initial appointment and action plan for working together moving forward. The intro call is also a great time to pinpoint minor "to-do" tasks for all parties (such as gathering a specific medical record, etc.) that will support our first "formal session” in being most effective.

Edwards, CO

Mental health and medical provider for adults (18+) at facility for SUD (substance use disorder), Mental Health and co-occurring conditions for residential and PHP (partial hospitalization) level patients. Independently conducted comprehensive H&Ps and psychiatric intakes for incoming patients from detox facilities, or through direct admissions. Extensively collaborated with psychiatrists, therapists, psychologists, nursing, registered dietitians and others, to provide and oversee comprehensive care. Scheduled with new intakes, as well as paneled/established patients (approx. 20–26) for weekly psych/medical appointments to assess status, order tests, initiate new medications, titrate/ taper doses, review labs and pharmacogenomic testing, refer to HBOT, TMS, Ketamine and/or SGB injections. Daily follow, making recommendations for immediate and ongoing plans of care after discharge.
