Dr. Beverly P. Horowitz is a doctorally educated, NYS licensed occupational therapist, social worker, and care manager who provides individualized, care management services. She is a Fellow of the American Occupational Therapy Association and a Fellow of the Gerontological Society of America. Dr. Horowitz provides strength-based care management services for adults experiencing impaired health, frailty, and/or disability. Services include clinical and home assessment, care planning , care coordination, home safety and fall prevention programs, individual and family education and counseling, care coordination, assistance with long term care needs, advocacy, and dementia care to help individuals maximize abilities and engage in desired activities for quality of life.
