Susan Nolan, MSN, RN is the founder of Discover Health and a registered nurse with over 25 years of experience in the nursing field. She decided to start Discover Health when she realized the need for personalized, patient-first care that also addresses the needs of family members. Her time working with doctors and hospitals prepared her for this opportunity. As a registered nurse, she skillfully manages the coordination and transitioning of patients from hospital to home or higher levels of care. During her time as a clinical nurse manager, she was responsible for the collaboration of an interdisciplinary team of physical, occupational, and speech therapists, social workers, registered nurses, home health aides, and insurance providers. She improved patient care by implementing new communication processes and procedures to quickly meet patient demand for services. She was also the Director of Clinical Operations/Director of Nursing. In this position, she was accountable for the oversight of day-to-day operations of patient care provided by physicians, and inter-disciplinary clinical teams, regulatory accreditation, revenue cycle, and budget. She headed clinical service line projects to improve the quality of the patient experience and timely access to care. Susan also developed an educational curriculum to support clinicians in new and existing operations.
