Meet Tammy Porter

Dr. Porter is a doctoral-prepared nurse with 31 years of experience specializing in care coordination and medical navigation.

About

Dr. Porter most recently served as a Senior Performance Partner of Strategy, Innovation and Population Health (SIPH) in the QUEST Collaborative at Premier, Inc. of Charlotte, NC. She has been responsible for partnering nationally with over 35 hospitals and health systems to improve patient safety and quality outcomes. She and the QUEST team provided consultative assistance to improve performance in such areas as healthcare-acquired conditions (HAC) and Patient Safety Indicators (PSI) as well as readmissions and overall mortalities. A focus for this collaborative also included the examination of social determinants of health by zip code for healthcare organizations and encouraged race and ethnicity inclusion in examining quality outcomes. Prior to joining Premier, Dr. Porter served as Executive Director of Quality for a health system in Southeast Missouri and has also served as Director of Case Management in California and Missouri. She has over 30 years nursing experience in healthcare which includes clinical care in obstetrics, ICU, home health and case management. She has also served in consultative roles for health systems in physician peer review, risk management, and executive leadership. Throughout her career, she has received awards for her outstanding service and professionalism in nursing, quality management, and as an outstanding clinical nursing student. She has served in numerous voluntary roles including Leadership Committee of The Mama's House Crisis Pregnancy Center, Board Member-Miss Illinois Scholarship Association, and Co-Chair of a Parish Nursing Committee. She has presented for the Missouri League of Nursing on CMS and the Affordable Care Act, developed her own Patient Advocacy business, and provided independent healthcare consulting for physicians, patients and attorneys. Most recently, Dr. Porter had her DNP manuscript published for Improving Care of Skilled Nursing Patients: Implementation of Early Sepsis Recognition in the Journal of Gerontological Nursing. *Note: Excluded services for substance abuse and mental health
